Annual Medical Form
We need to have an Annual Medical Form on file for everyone who participates in Pack 369 events - not just your Scout, but parents and tagalongs as well. Please print out as many copies as you need, and return the completed form to Amy Payne. Note that you only need to fill in parts A and C of the form; we do not require the physician's part at this time.
